Services include general dentistry in the form of x-rays, fillings and extractions.

Appointments are required and can be made by calling the dental program.

Our Vision
To provide Nassau County residents, who qualify as low income program participants, adequate care for acute or chronic dental issues.

Dental Personnel
One dentist, a dental coordinator, two dental assistants, and a dental hygienist.

Eligibility Process

• Patient must be 21 years of age or older
• Patients must be Nassau County residents
• Photo identification is required
• Social Security cards are required for every member of the family
• Proof of income for the entire household is required.
• Patients must bring medicaid card, if applicable

Dental Hygiene Services:

Patients who have Medicaid are eligible for services through the dental program's self-pay program. This means that patients will be held responsible for the payment of all services they receive. Please refer to the fee schedule below for current rates. Note that the dental program does NOT bill Medicare for any services provided, and Medicaid is not responsible for any fees incurred.
